  • Reduction of cholesterol concentrations in renal transplantation patients

Pharmacology

  • Selective inhibitor of HMG-CoA reductase
  • Lowers plasma cholesterol and lipoprotein levels
  • Inhibits cholesterol synthesis in the liver
  • Increases hepatic LDL receptors for enhanced uptake of LDL
  • Rapidly absorbed after oral administration
  • Maximum plasma concentrations occur within 1 to 2 hours
  • Metabolized to ortho- and parahydroxylated derivatives
  • Excreted primarily in bile
  • Half-life of inhibitory activity for HMG-CoA reductase is 20 to 30 hours

Dosage & Administration

  • Adults initial dose: 10 mg once daily
  • Adults maximum dose: 80 mg once daily
  • Children (10-18 years) initial dose: 10 mg once daily
  • Children (10-18 years) maximum dose: 20 mg once daily
  • Patient should be placed on a standard cholesterol-lowering diet before receiving Avas

Interactions

  • Increased risk of myopathy with concurrent administration of certain drugs
  • Plasma concentrations affected by coadministration with antacid, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin

Contraindications

  • Hypersensitivity to any component of Avas
  • Active liver disease
  • Unexplained persistent elevations of serum transaminases
  • History of serious adverse reaction to prior HMG-CoA reductase inhibitors

Pregnancy & Lactation

  • Potential for serious adverse reactions during breastfeeding

Precautions & Warnings

  • Liver function tests should be performed before initiation of treatment and periodically thereafter
  • Avas should be used with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
  • Avas therapy should be discontinued if markedly elevated CPK levels occur or myopathy is diagnosed or suspected

Use in Special Populations

  • Plasma concentrations higher in healthy elderly subjects
  • No clinically significant difference in LDL-C reduction between men and women
  • Renal disease has no influence on plasma concentrations
  • Hemodialysis not expected to significantly enhance clearance of Avas
  • Plasma concentrations markedly increased in patients with chronic alcoholic liver disease

Overdose Effects

  • Specific treatment not available for Avas overdose
  • Symptomatic treatment and supportive measures instituted
  • Liver function tests should be performed
  • Serum CK levels should be monitored
