Indications
- Reduce total cholesterol and LDL cholesterol in patients with familial hypercholesterolaemia
- Treat mixed dyslipidemia and hypertriglyceridemia
- Reduce serum triglyceride levels in hypertriglyceridaemia
- Treat dysbetalipoproteinaemia
- Reduce cardiac ischaemic events in patients with coronary artery disease
- Lower cholesterol levels in patients with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Inhibits cholesterol synthesis in the liver
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
- Rapidly absorbed after oral administration
- Metabolized to ortho- and parahydroxylated derivatives
Dosage & Administration
- Adults dosage range: 10-80 mg once daily
- Child dosage range: 10-20 mg once daily
- Initial dose for familial hypercholesterolaemia: 10 mg daily
- Maximum dose for hypercholesterolaemia: 80 mg once daily
Interaction
- Increased risk of myopathy with cyclosporine
- Plasma concentrations decrease when coadministered with antacid
- Plasma concentrations decrease when coadministered with colestipol
- Steady-state plasma digoxin concentrations increase with digoxin coadministration
- Increased plasma concentrations with erythromycin
- Increased AUC values for oral contraceptives
Contraindications
- Hypersensitivity to any component
- Active liver disease
- History of serious adverse reaction to HMG-CoA reductase inhibitors

- Discontinue therapy if markedly elevated CPK levels occur
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects
- No clinically significant difference in LDL reduction between men and women
- No dose adjustment needed in renal dysfunction
- Markedly increased concentrations in chronic alcoholic liver disease
