Barif 40 mg (Tablet)

Indications

• Chronic management of hyperuricemia in patients with gout
• Not recommended for the treatment of asymptomatic hyperuricemia

Pharmacology

• Febuxostat is a non-purine, selective xanthine oxidase (XO) inhibitor
• Decreases serum uric acid level by inhibiting xanthine oxidase
• Not expected to inhibit other enzymes involved in purine and pyrimidine synthesis

Dosage & Administration

• Recommended starting dose of Febuxostat 40 mg tablet once daily
• Different dosages based on serum uric acid levels and specific conditions
• No dose adjustment necessary for renal impairment and mild to moderate hepatic impairment

Interaction

• Contraindicated with mercaptopurine, azathioprine, and theophylline
• Possible interactions with drugs like rosiglitazone, naproxen, colchicine, indometacin, and warfarin
• Delay in absorption when taken with antacids containing magnesium hydroxide and aluminum hydroxide

Contraindications

• Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline

Side Effects

• Most commonly reported adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and oedema
• Rare serious hypersensitivity reactions observed

Pregnancy & Lactation

• Pregnancy Category C: Use during pregnancy only if the potential benefit justifies the potential risk to the fetus
• Caution should be exercised when administered to a nursing woman

Precautions & Warnings

• Increase in gout flares during initiation of treatment
• Higher rate of cardiovascular thromboembolic events observed
• Transaminase elevations have been observed in Barif-treated patients

Overdose Effects

• Studied in healthy subjects in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
