- Reduction of cholesterol concentrations associated with diabetes mellitus
- Reduction of cholesterol concentrations associated with renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
- Rapidly absorbed after oral administration
- Metabolized to ortho- and parahydroxylated derivatives
- Inhibition of HMG-CoA reductase by metabolites
- Excreted primarily in bile
- Mean plasma elimination half-life approximately 14 hours
Dosage & Administration
- Initial dosage of 10 mg once daily for adults
- Maximum dosage of 80 mg once daily for adults
- Initial dosage of 10 mg once daily for children (10-18 years)
- Maximum dosage of 20 mg once daily for children (10-18 years)
Interactions
- Increased risk of myopathy with cyclosporine
- Decreased plasma concentrations with antacid coadministration
- Enhanced LDL-C reduction with coadministration of colestipol
- Increased steady-state plasma digoxin concentrations with coadministration of digoxin
- Increased plasma concentrations with coadministration of erythromycin
- Increased AUC values for oral contraceptives with coadministration of oral contraceptive
- No clinically significant effect on prothrombin time when administered to patients receiving chronic warfarin treatment
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease
- Unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy & Lactation
- Contraindicated during pregnancy

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment
- Caution in patients who consume substantial quantities of alcohol
- Caution in patients with history of liver disease
- Therapy should be discontinued if markedly elevated CPK levels occur or myopathy is diagnosed
Use in Special Populations
- Higher plasma concentrations in elderly subjects
- Greater LDL-lowering in elderly patients
- No clinically significant difference between men and women
- No influence on plasma concentrations or LDL-C reduction in renal disease
- No significant enhancement of clearance with hemodialysis
- Markedly increased plasma concentrations in chronic alcoholic liver disease
Overdose Effects
- Specific treatment not available
- Symptomatic treatment and supportive measures required
- Liver function tests and serum CK levels should be monitored
- Hemodialysis not expected to significantly enhance clearance
