Indications
- Chronic management of hyperuricemia in gout patients
Pharmacology
- Non-purine, selective xanthine oxidase inhibitor
- Decreases serum uric acid levels
- Inhibits xanthine oxidase enzyme responsible for uric acid production
Dosage & Administration
- Recommended starting dose of 40mg tablet once daily
- Increase to 80mg if serum uric acid not less than 6mg/dL after 2 weeks
- Increase to 120mg if serum uric acid not less than 6mg/dL after 2-4 weeks
- Special dose for Tumor Lysis Syndrome
- Prophylaxis for gout flares
- No dose adjustment for mild to moderate renal impairment
- No dose adjustment for mild to moderate hepatic impairment
- Not recommended for pediatric use
Interaction
- Not recommended with Mercaptopurine/Azathioprine
- Co-administration with Rosiglitazone/CYP2C8 substrates might not require dose adjustment
- Co-administration with Naproxen and other glucuronidation inhibitors might not require dose adjustment
- Monitoring needed for inducers of glucuronidation
- Co-administration with Colchicine/Indometacin/Hydrochlorothiazide/Warfarin might not require dose adjustment
- Potential delay in absorption when taken with antacids
Contraindications
- Contraindicated in patients treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Common adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Caution during lactation
Precautions & Warnings
- Increased gout flares during initiation of anti-hyperuricemic agents
- Monitor for cardiovascular events
- Monitor liver function tests periodically
Overdose Effects
- Studied in healthy subjects up to 300mg daily for seven days without dose-limiting toxicities
