Indications
- Chronic management of hyperuricemia in patients with gout
Pharmacology
- Non-purine selective xanthine oxidase inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
Dosage and Administration
- Recommended starting dose of 40 mg tablet once daily
- Dosage adjustment based on serum uric acid level
- Management for tumor lysis syndrome
- Gout flare management
- No dose adjustment for renal impairment
- No dose adjustment for mild to moderate hepatic impairment
Interaction
- Inhibition of XO by Feluric may cause increased plasma concentrations of mercaptopurine/azathioprine
- Co-administration of Feluric with rosiglitazone or other CYP2C8 substrates
- Effect of NSAIDs and probenecid on Feluric metabolism
- Effect of potent inducers of UGT enzymes on Feluric metabolism
- Co-administration of Feluric with colchicine or indomethacin
- Effect of antacids on the absorption of Feluric
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Common adverse reactions - gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, oedema
- Rare serious hypersensitivity reactions observed
Pregnancy and Lactation
- Pregnancy Category C
- Caution during nursing
Precautions and Warnings
- Increase in gout flares during initiation of treatment
- Higher rate of cardiovascular thromboembolic events observed
- Monitoring for signs and symptoms of MI and stroke
- Monitor liver function tests periodically
Overdose Effects
- Studied in healthy subjects in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
