Indications
- Reduction of elevated total cholesterol levels

Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
- Rapid absorption after oral administration
- Metabolized to ortho- and parahydroxylated derivatives and beta-oxidation products
- Extensively metabolized by cytochrome P450 3A4
- Eliminated primarily in bile
- Mean plasma elimination half-life of approximately 14 hours
Dosage & Administration
- Adult dosage: 10-80 mg once daily
- Child (10-18 years) dosage: 10-20 mg once daily
- Initial dosage of 10 mg daily for familial hypercholesterolaemia
- Dosage adjustment according to lipid levels
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, and azole antifungals
- Altered plasma concentrations when coadministered with antacids, colestipol, digoxin, erythromycin, and oral contraceptives
- No clinically significant effect on prothrombin time when coadministered with warfarin
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy & Lactation

Precautions & Warnings
- Liver function tests before initiation of treatment and periodically thereafter
- Discontinue if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects
- Greater degree of LDL-lowering in the elderly patient population
- No clinically significant difference in LDL-C reduction between men and women
- No influence on plasma concentrations or LDL-C reduction in renal disease
