Indications
- Chronic management of hyperuricemia in patients with gout
Pharmacology
- Non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
Dosage & Administration
- Recommended starting dose: Febuxostat 40 mg tablet once daily
- For patients not achieving serum uric acid less than 6 mg per dL after 2 weeks, increase to 80 mg tablet once daily
- For tumor lysis syndrome: 120 mg tablet once daily
- Gout flares may occur after initiation, use NSAID or colchicine for prophylaxis
- No dose adjustment for mild to moderate renal impairment
- No dose adjustment for mild to moderate hepatic impairment
- Safety and effectiveness in pediatric patients under 18 years not established
Interaction
- Contraindicated with mercaptopurine/azathioprine
- Co-administration with rosiglitazone or other CYP2C8 substrates is not expected to require dose adjustment
- Can be co-administered with naproxen with no dose adjustment
- Potent inducers of UGT enzymes might lead to increased metabolism and decreased efficacy
- Can be co-administered with colchicine or indomethacin with no dose adjustment
- No dose adjustment necessary for hydrochlorothiazide or warfarin
- Concomitant ingestion of antacid containing magnesium hydroxide and aluminium hydroxide delays absorption
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Common adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Caution advised during lactation
Precautions & Warnings
- Increased gout flares observed during initiation, prophylactic therapy may be beneficial
- Monitor for signs and symptoms of MI and stroke
- Monitor liver function tests periodically
Overdose Effects
- Studied in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
