Indications
- Chronic management of hyperuricemia in patients with gout
Pharmacology
- Febuxostat is a non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
Dosage & Administration
- Recommended starting dose of Febuxostat 40 mg tablet once daily
- For patients not achieving serum uric acid less than 6 mg per dL after 2 weeks, 80 mg tablet once daily is recommended
- For patients not achieving serum uric acid less than 6 mg per dL after 2-4 weeks with 80mg tablet, then 120 mg tablet once daily is recommended
- Dose for Tumor Lysis Syndrome is one 120 mg tablet once daily
- Gout flares management with non-steroidal anti-inflammatory drug (NSAID) or colchicine is recommended
- No dose adjustment necessary in patients with mild to moderate renal impairment
- No dose adjustment necessary in patients with mild to moderate hepatic impairment
- Safety and effectiveness in pediatric patients under 18 years of age have not been established
Interaction
- Concomitant use of Mercaptopurine/Azathioprine not recommended
- Co-administration with rosiglitazone or other CYP2C8 substrates is not expected to require dose adjustment
- No dose adjustment necessary for co-administration with naproxen, colchicine, indomethacin, hydrochlorothiazide, or warfarin
- Concomitant ingestion of antacid containing magnesium hydroxide and aluminum hydroxide may cause delayed absorption of Febuxostat
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Most commonly reported adverse reactions are gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
Pregnancy & Lactation
- Pregnancy Category C
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Increased gout flares observed during initiation of treatment
- Higher rate of cardiovascular thromboembolic events observed in patients treated with Febuxostat
- Monitor liver function tests periodically
Overdose Effects
- Febustat studied in healthy subjects in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
