Indications
- Management of hyperuricemia in gout patients
Pharmacology
- Non-purine xanthine oxidase inhibitor
- Decreases serum uric acid level
- Inhibition of xanthine oxidase
- Does not inhibit other enzymes
Dosage & Administration
- Recommended starting dose of 40 mg tablet once daily
- May increase to 80 mg or 120 mg if necessary
- Dose for tumor lysis syndrome
- Management of gout flares
- No dose adjustment for renal or hepatic impairment
- Not established for pediatric use
Interactions
- Not recommended with mercaptopurine/azathioprine
- No dose adjustment with rosiglitazone/CYP2C8 substrates
- Potential impact with naproxen and other inhibitors of glucuronidation
- Monitoring recommended with inducers of glucuronidation
- Can be co-administered with colchicine/indometacin/hydrochlorothiazide/warfarin
- Delayed absorption with antacids
Contraindications
- Patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Commonly reported reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Caution advised during nursing

- Increased gout flares during initiation of treatment
- Monitoring for cardiovascular events
- Periodic liver function test monitoring
