Indications
- Adjunct to diet for reducing cholesterol and triglycerides levels
- Treatment for hypercholesterolaemia and familial hypercholesterolaemia
- Reducing cardiac ischaemic events in patients with coronary artery disease
- Treatment for dyslipidemia and dysbetalipoproteinaemia
- Lowering cholesterol levels in patients with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake of LDL
- Rapidly absorbed after oral administration
- Metabolized to ortho- and parahydroxylated derivatives
- Eliminated primarily in bile
Dosage & Administration
- Adult dosage usually starts at 10 mg once daily
- Children's dosage starts at 10 mg once daily, with max dosage of 20 mg once daily
- Dosage adjustment based on lipid level analysis within 2 to 4 weeks
- Can be administered with or without food
Interaction
- Increased risk of myopathy with concurrent administration of certain drugs
- Plasma concentrations decrease with coadministration of antacids and increase with coadministration of erythromycin
- Impact on plasma concentrations of digoxin, oral contraceptives, and warfarin
- Antacid and colestipol affect plasma concentrations but not LDL-C reduction
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment
- Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in Special Populations
- Plasma concentrations in healthy elderly subjects are higher
- No clinically significant difference in LDL-C reduction between men and women
- Renal disease has no influence on plasma concentrations or LDL-C reduction
- Not expected to significantly enhance clearance with hemodialysis
Overdose Effects
- Specific treatment not available
- Symptomatic treatment recommended with monitoring of liver function and serum CK levels
- Hemodialysis not expected to significantly enhance clearance
