Indications
- Spasticity resulting from multiple sclerosis
- Flexor spasms and concomitant pain
- Clonus and muscular rigidity
- Skeletal muscle spasm resulting from rheumatic disorders
- Spinal cord injuries and other spinal cord diseases
- Cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- Inhibits monosynaptic and polysynaptic reflexes at the spinal level
- Stimulates GABA receptors
- Inhibits the release of glutamate and aspartate
- Produces CNS depression
- Exerts an antinociceptive effect
Dosage & Administration
- Adult & children over 10 years: 5 mg 3 times daily, gradually increased; max. 100 mg daily
- Children <10 years: Treatment usually started with 2.5 mg given 4 times daily
- Doses based on age: 12 month-2 years: 10-20 mg, 2 years-6 years: 20-30 mg, 6 years-10 years: 30-60 mg
Interaction
- Increased sedation with agents acting on the central nervous system, alcohol, or synthetic opiates
- Increased fall in blood pressure with antihypertensives
- Potentiation with tricyclic antidepressants resulting in pronounced muscular hypotonia
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Central Nervous System: Headache, insomnia, euphoria, depression, confusion, hallucinations, paraesthesia, nightmares, muscle pain, tremor, rigidity, ataxia, blurred vision, seizures, respiratory depression
- Cardiovascular: Hypotension, dyspnoea, palpitation, chest pain, syncope
- Gastrointestinal: Nausea, constipation, dry mouth, vomiting, diarrhea
- Genitourinary: Urinary frequency, enuresis, urinary retention
- Other: Rash, pruritus, ankle edema, weight gain, visual disturbances, hepatic function disorders, increase in spasticity
Pregnancy & Lactation
- Pregnancy category B3
- Safe use during pregnancy not established
- Crosses the placental barrier
- Excreted in breast milk (small quantities)
Precautions & Warnings
- Lower doses for patients with impaired renal function
- Cautious treatment for patients with psychotic disorders, schizophrenia, depressive or manic disorders
- Use with caution in patients with epilepsy, peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- Monitoring of respiratory and cardiovascular function is essential
- Neurogenic disturbances affecting bladder emptying may improve, but acute retention of urine may occur
Overdose Effects
- Gastric lavage is important in case of severe overdose
