Indications
- spasticity resulting from multiple sclerosis
- flexor spasms and concomitant pain
- clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries and other spinal cord diseases
- cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- Baclofen inhibits both monosynaptic and polysynaptic reflexes at the spinal level by stimulating the GABA B receptors
- Baclofen exerts an antinociceptive effect
Dosage and administration
- 5 mg 3 times daily for adults and children over 10 years
- gradually increased; max. 100 mg daily
- Children <10 years: Treatment is usually started with 2.5 mg given 4 times daily then raised according to requirement
- Daily maintenance dose varies by age range
Interaction
- Increased sedation with agents acting on the central nervous system, alcohol or synthetic opiates
- Increased fall in blood pressure with combined treatment with antihypertensives
- Potential potentiation of pharmacological effects with tricyclic antidepressants
- Reports of mental confusion, hallucinations, headaches, nausea and agitation with levodopa and carbidopa
- Increased CNS depressant effects with concurrent use of MAO inhibitors
- Caution needed when administering with magnesium sulphate or other neuromuscular blocking agents

- Hypersensitivity to any component of this product

Pregnancy and lactation
- Pregnancy category B3
- Baclofen crosses the placental barrier
- Excretion in breast milk with no expected undesirable effects on the infant
Precautions and warnings
- Lower doses for impaired renal function or chronic haemodialysis
- Cautious treatment for patients with psychotic disorders, schizophrenia, depressive or manic disorders
- Potential lowering of convulsion threshold in epilepsy patients
- Caution in patients with various conditions including peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
- Careful monitoring of respiratory and cardiovascular function essential
- Possible improvements in neurogenic disturbances affecting bladder emptying
- Poor tolerance in stroke patients
- Periodic laboratory tests in patients with hepatic diseases or diabetes mellitus
Overdose effects
- Gastric lavage is important in case of severe overdose
