Indications
- spasticity resulting from multiple sclerosis
- flexor spasms and concomitant pain, clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries and other spinal cord diseases
- cerebrovascular accidents or neoplastic or degenerative brain disease
Pharmacology
- Inhibition of monosynaptic and polysynaptic reflexes
- Stimulation of GABA receptors
- Inhibition of glutamate and aspartate release
- Antinociceptive effect
- CNS depression
Dosage & Administration
- Recommended adult dosage: 5 mg 3 times daily
- Gradual dosage increase up to max. 100 mg daily
- Children's dosage based on age and weight
- Administration with or after food
Interaction
- Potential for increased sedation when combined with CNS agents, alcohol, or synthetic opiates
- Likely interaction with antihypertensives causing increased blood pressure fall
- Potentiation of effects when combined with tricyclic antidepressants
- Reports of mental confusion, hallucinations, headaches, nausea, and agitation when combined with levodopa and carbidopa
- Possible combined effects with MAO inhibitors and neuromuscular blocking agents
Contraindications
- Patients with hypersensitivity to any component
Side Effects
- Transient drowsiness
- Daytime sedation
- Dizziness
- Weakness
- Fatigue
- Adverse reactions in the central nervous system, cardiovascular, gastrointestinal, genitourinary, and other systems
Pregnancy & Lactation
- Pregnancy category B3
- Safety in pregnancy not fully established
- Excretion in breast milk with minimal expected effects on the infant
Precautions & Warnings
- Lower doses for patients with impaired renal function or undergoing chronic hemodialysis
- Cautious use in patients with psychotic disorders, schizophrenia, depressive or manic disorders, or confusional states
- Possible lowering of convulsion threshold in patients with epilepsy
- Caution in patients with peptic ulcers, cerebrovascular diseases, hepatic, renal, or respiratory failure
- Need for careful respiratory and cardiovascular monitoring, especially in patients with cardiopulmonary disease and respiratory muscle weakness
- Caution in patients with preexisting sphincter hypertonia
- Poor tolerance in stroke patients
- Periodic laboratory tests for patients with hepatic diseases or diabetes mellitus
Overdose Effects
- Importance of gastric lavage in case of severe overdose
