Indications
- Reduction of elevated total cholesterol
- Reduction of LDL cholesterol
- Reduction of apolipoprotein B (Apo-B) levels
- Reduction of triglycerides levels
- Treatment of heterozygous and homozygous familial hypercholesterolaemia
- Treatment of mixed dyslipidemia (Fredrickson Type Ia and Ib)
- Treatment of hypertriglyceridaemia (Fredrickson Type IV)
- Treatment of dysbetalipoproteinaemia (Fredrickson Type III)
- Reduction of cardiac ischaemic events
- Treatment of hypercholesterolemia associated with diabetes mellitus or renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowering of plasma cholesterol and lipoprotein levels
- Enhanced uptake and catabolism of LDL
- Rapid absorption after oral administration
- Distribution: Mean volume of distribution is approximately 381 liters
- Metabolism via cytochrome P450 3A4
- Excretion via bile
- Plasma elimination half-life in humans is approximately 14 hours
Dosage & Administration
- Recommended starting dose of 10 or 20 mg once daily
- Maximum dosage range of 10 to 80 mg once daily
- Administered as a single dose at any time of the day
- Pediatric dosage starting at 10 mg/day with maximum recommended dose of 20 mg/day
- Doses should be individualized according to the recommended goal of therapy
Interactions
- Increased risk of myopathy with concurrent administration of certain drugs
- Altered plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, or oral contraceptives
- Coadministration with warfarin had no clinically significant effect on prothrombin time
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Use in Special Populations
- Higher plasma concentrations in elderly subjects
- No clinically significant difference in LDL-C reduction between men and women
- No dose adjustment necessary in patients with renal dysfunction
- Markedly increased plasma concentrations in patients with chronic alcoholic liver disease
