Indications
- spasticity resulting from multiple sclerosis
- flexor spasms
- concomitant pain
- clonus and muscular rigidity
- skeletal muscle spasm resulting from rheumatic disorders
- spinal cord injuries
- other spinal cord diseases
- cerebrovascular accidents
- neoplastic or degenerative brain disease
Pharmacology
- inhibits monosynaptic and polysynaptic reflexes
- stimulates GABA receptors
- inhibits release of glutamate and aspartate
- acts at intraspinal sites producing CNS depression
- exerts antinociceptive effect
Dosage & Administration
- 5 mg 3 times daily for adults & children over 10 years
- max. 100 mg daily for adults & children over 10 years
- gradually increased dosage
- maintenance dose based on age for children under 10 years
Interaction
- increased sedation with agents acting on CNS, alcohol, or synthetic opiates
- risk of respiratory depression
- increase in blood pressure when combined with antihypertensives
- potentiation of effects with tricyclic antidepressants
- reports of mental confusion with Parkinson's disease treatment
- increased CNS depressant effects with MAO inhibitors
- caution with magnesium sulfate or neuromuscular blocking agents

Pregnancy & Lactation
- category B3
- safe use not established during pregnancy
- used when potential benefits outweigh possible hazards

Precautions & Warnings
- lower doses for patients with impaired renal function
- caution for patients with psychotic disorders, schizophrenia, depressive or manic disorders
- careful use in patients with epilepsy
- caution in patients with peptic ulcers, cerebrovascular diseases, or hepatic, renal, or respiratory failure
- monitoring of respiratory and cardiovascular function essential
- caution in patients with preexisting sphincter hypertonia
- no significant benefit for stroke patients
- appropriate laboratory tests for hepatic diseases or diabetes mellitus
Overdose Effects
- gastric lavage is important in case of severe overdose
