Pharmacological Action
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors
Absorption
- Rapidly absorbed after oral administration
- Maximum plasma concentrations within 1 to 2 hours
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives
- Metabolized by cytochrome P450 3A4
Distribution
- Approximately 98% bound to plasma proteins
Excretion
- Eliminated primarily in bile
Dosage and Administration
- Adults: Usually 10 mg once daily; may be increased to max 80 mg once daily
- Child (10-18 years): Initially 10 mg once daily, increased if necessary to max 20 mg once daily
Interaction with Other Drugs
- Concurrent administration with certain drugs increases risk of myopathy
- Plasma concentrations altered when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, or warfarin
Contraindications
- Hypersensitivity to any component
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

- Use with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
Use in Special Populations
- Plasma concentrations are higher in healthy elderly subjects
- Pharmacokinetic data in the pediatric population not available
- No clinically significant difference in LDL-C reduction between men and women
