Indications
- Adjunct to diet to reduce elevated total cholesterol levels
- Reduce LDL cholesterol in patients with hypercholesterolaemia
- Reduce triglycerides levels in patients with dyslipidemia
- Reduce cardiac ischaemic events in patients with coronary artery disease
- Reduce cholesterol levels in patients with diabetes mellitus
- Reduce cholesterol levels in patients with renal transplantation
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake of LDL
- Rapidly absorbed after oral administration
- Metabolized to ortho- and parahydroxylated derivatives
Dosage & Administration
- Recommended starting dose of 10 or 20 mg once daily
- Dose adjustment based on lipid levels analysis
- Maximum dose of 80 mg once daily
- Can be taken with or without food
- Adjustments should be made at intervals of 4 weeks or more
Interactions
- Increased risk of myopathy with cyclosporine
- Plasma concentrations decreased with antacid coadministration
- Plasma concentrations decreased with colestipol coadministration
- Steady-state plasma digoxin concentrations increased with digoxin coadministration
- Plasma concentrations increased with erythromycin coadministration
- AUC values increased with oral contraceptive coadministration
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to HMG-CoA reductase inhibitors

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment
- Discontinue therapy if markedly elevated CPK levels occur
- Use with caution in patients with substantial alcohol consumption
- Use with caution in patients with history of liver disease
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects
- No significant difference in LDL-C reduction between men and women
- Renal disease does not influence plasma concentrations
- Clinical data suggest greater LDL-lowering in elderly patients
Overdose Effects

- Symptomatic treatment and supportive measures
- Liver function tests should be performed
- Serum CK levels should be monitored
- Hemodialysis not expected to significantly enhance clearance
