Description

  • G-Atorvast is used as an adjunct to diet to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B, and triglycerides levels in various conditions such as familial hypercholesterolaemia, mixed dyslipidemia, hypertriglyceridaemia, dysbetalipoproteinaemia, and asymptomatic or mild to moderate symptomatic coronary artery disease with elevated LDL-cholesterol level.

Dosage Administration

  • Adults: Usually 10 mg once daily, may be increased to max. 80 mg once daily based on response. Children (10-18 years): Initially 10 mg once daily, increased if necessary to max. 20 mg once daily.

Pharmacology

  • Atorvastatin is a selective inhibitor of HMG-CoA reductase, lowering plasma cholesterol and lipoprotein levels by inhibiting cholesterol synthesis in the liver and increasing the number of hepatic LDL receptors.

Mode of Action

  • Atorvastatin is absorbed rapidly after oral administration, with maximum plasma concentrations occurring within 1 to 2 hours. It is extensively metabolized and eliminated primarily in bile following hepatic and/or extra-hepatic metabolism.

Interaction

  • The risk of myopathy is increased with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals, antacid, colestipol, digoxin, oral contraceptives, and warfarin.

Contraindications

  • Hypersensitivity to any component of the medication, active liver disease or unexplained persistent elevations of serum transaminases, and history of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors.

Side Effects

  • Constipation, flatulence, dyspepsia, abdominal pain, infection, headache, back pain, rash, asthenia, arthralgia, myalgia.

Pregnancy Lactation

  • Contraindicated during pregnancy and breastfeeding due to safety concerns and potential adverse effects on fetal and infant health.

Precautions Warnings

  • Liver function tests should be performed before initiation of treatment and regularly thereafter. Use with caution in patients with substantial alcohol consumption or history of liver disease.

Use in Special Populations

  • Plasma concentrations of Atorvast are higher in healthy elderly subjects than in young adults, with similar LDL-lowering effects. Limited pharmacokinetic data available for pediatric population.

Overdose Effects

  • Specific treatment not available for Atorvastatin overdose. Symptomatic and supportive measures should be instituted, with monitoring of liver function tests and serum CK levels.

  • Keep in a dry place away from light and heat, and out of the reach of children.
