Indications
- Adjunct to diet to reduce elevated total cholesterol
- LDL cholesterol reduction
- Triglycerides level reduction
- Treatment for familial hypercholesterolaemia

Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors
- Rapid absorption after oral administration
- Metabolized to ortho- and parahydroxylated derivatives
- Mainly eliminated in bile
- Long plasma elimination half-life
Dosage & Administration
- Adult dosage: 10-80 mg once daily
- Child dosage: 10-20 mg once daily
- Dose adjustment based on lipid levels within 2-4 weeks of initiation
- Can be taken with or without food
Interactions
- Increased risk of myopathy with concurrent cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
- Effects of antacids, colestipol, digoxin, erythromycin, oral contraceptives, warfarin on atorvastatin levels and action
Contraindications
- Hypersensitivity to any component
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Precautions & Warnings
- Liver function tests before initiation and periodically thereafter
- Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed
Use in Special Populations
- High plasma concentrations in healthy elderly subjects
- No clinically significant difference in LDL-C reduction between genders
- No dose adjustment necessary in renal insufficiency
- Markedly increased plasma concentrations in chronic alcoholic liver disease

- Symptomatic and supportive measures required
- Liver function tests and serum CK levels monitoring recommended
