Description

  • Selective inhibitor of HMG-CoA reductase
  • Used for reducing total cholesterol and LDL cholesterol levels
  • Adjunct to diet for reducing elevated cholesterol and triglycerides levels
  • Treatment for hypercholesterolaemia, hypertriglyceridaemia, and dyslipidemia
  • Reduces risk of cardiac ischaemic events
  • Adjunct to non-pharmacological measures
  • Contraindicated in active liver disease
  • Not recommended during pregnancy or lactation
  • Precaution in patients with substantial alcohol consumption or history of liver disease

Dosage & Administration

  • Adult dosage: 10-80 mg once daily
  • Child dosage (10-18 years): 10-20 mg once daily
  • Initial dosage is 10 mg once daily, may be adjusted as per response
  • Should be taken with or without food
  • Analysis of lipid levels within 2-4 weeks after initiation

Pharmacological Action

  • Inhibits HMG-CoA reductase
  • Lowers plasma cholesterol and lipoprotein levels

Absorption

  • Rapidly absorbed after oral administration
  • Maximum plasma concentrations occur within 1 to 2 hours

Distribution

  • Mean volume of distribution approximately 381 liters
  • 98% bound to plasma proteins

Metabolism

  • Extensively metabolized to ortho- and parahydroxylated derivatives
  • In vitro inhibition of HMG-CoA reductase by metabolites

Excretion

  • Eliminated primarily in bile
  • Mean plasma elimination half-life is approximately 14 hours

Interaction

  • Risk of myopathy with concurrent administration of specific drugs
  • Plasma concentration alteration when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, or warfarin

Contraindications

  • Hypersensitivity to any component of the medication
  • Active liver disease or unexplained persistent elevations of serum transaminases
  • History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

  • Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed/suspected

Use in Special Populations

  • Higher plasma concentrations in healthy elderly subjects
  • Greater degree of LDL-lowering in the elderly
  • No clinically significant difference in LDL-C reduction between men and women
  • Renal disease has no influence on plasma concentrations or LDL-C reduction
  • Hemodialysis not expected to significantly enhance clearance of the medication
  • Markedly increased plasma concentrations in patients with chronic alcoholic liver disease

Overdose Effects

  • No specific treatment available
  • Symptomatic treatment and supportive measures instituted
  • Liver function tests and serum CK levels should be monitored
