Indications

Pharmacology
- Mediates hypersensitivity and allergic reactions
- Responsible for degranulation of mast cells and basophils
- Release of histamine and other inflammatory mediators
- Histamine binds to H1 receptors activating downstream effects
- Fexofenadine as an inverse agonist of the H1 receptor
- Potent and selective affinity for H1 receptors
- No evidence of other blocking activities
- Does not cross the blood-brain barrier
Dosage & Administration
- Recommended dosage for adults and children 12 years and older
- Different dosage for impaired renal function
- Tablet: 60 mg twice daily or 120 mg once daily or 180 mg once daily
- Suspension: 30 mg or 5 ml twice daily
Interaction
- Does not undergo hepatic biotransformation
- Coadministration with erythromycin or ketoconazole increases plasma level
- No interaction with omeprazole
- Antacid containing aluminium and magnesium hydroxide reduces bioavailability
Contraindications
- Contraindicated in patients with known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients
Side Effects
- Nervous system disorders: headache, drowsiness, dizziness
- Gastrointestinal disorders: nausea
- Immune system disorders: hypersensitivity reactions
- Psychiatric disorders: insomnia, nervousness, sleep disorders
- Cardiac disorders: tachycardia, palpitations
- Gastrointestinal disorders: diarrhea
- Skin and subcutaneous tissue disorders: rash, urticaria, pruritus
Pregnancy & Lactation
- Limited data from use in pregnant women
- No direct or indirect harmful effects in limited animal studies
- Not recommended for mothers breast-feeding their babies
- No human data on the effect on fertility
Precautions & Warnings
- Limited data in older people and renally or hepatically impaired patients
- Administered with care in special groups
- Antihistamines can be associated with adverse reactions in patients with cardiovascular disease
Use in Special Populations
- Pharmacokinetics altered in individuals with renal impairment
- Dose adjustment recommended in patients with decreased renal function
- No substantial effect on pharmacokinetics in moderate to severe hepatic disease
- Adverse events similar in elderly patients
Overdose Effects
- Reported effects: dizziness, drowsiness, fatigue, dry mouth
- Single doses and prolonged administration without clinically significant adverse reactions
- Maximum tolerated dose not established
- Symptomatic and supportive treatment recommended
