Indications

  • Reduction of cardiac ischemic events in patients with asymptomatic or mild to moderate symptomatic coronary artery disease with elevated LDL-cholesterol level
  • Reduction of total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with or exacerbated by diabetes mellitus or renal transplantation

Pharmacology

  • Lowers plasma cholesterol and lipoprotein levels
  • Rapid absorption after oral administration
  • Extensively metabolized to ortho- and parahydroxylated derivatives
  • Eliminated primarily in bile
  • Plasma elimination half-life approximately 14 hours

Dosage & Administration

  • Adults: Usually 10 mg once daily; may be increased to max. 80 mg once daily if necessary
  • Children (10-18 years): Initially 10 mg once daily, increased if necessary to max. 20 mg once daily

Interactions

  • Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
  • Decreased plasma concentrations of atorvastatin when coadministered with antacid suspension or colestipol
  • Increased plasma concentrations of digoxin, erythromycin, and oral contraceptives when coadministered with atorvastatin
  • No clinically significant effect on prothrombin time when coadministered with warfarin

Contraindications

  • Hypersensitivity to any component
  • Active liver disease or unexplained persistent elevations of serum transaminases
  • History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

  • Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed or suspected

Use in Special Populations

  • Plasma concentrations in healthy elderly subjects are higher than in young adults
  • No clinically significant difference in LDL-C reduction between men and women
  • Renal disease has no influence on plasma concentrations or LDL-C reduction
  • In patients with chronic alcoholic liver disease, plasma concentrations are markedly increased
