Description
A selective inhibitor of HMG-CoA reductase used to reduce elevated levels of cholesterol and triglycerides.
Dosage and administration
- Adults: 10-80 mg once daily
- Children (10-18 years): 10-20 mg once daily
- Initial dose for familial hypercholesterolaemia: 10 mg daily, increased to 40-80 mg once daily
Pharmacology
- Mode of action: Inhibits HMG-CoA reductase to lower plasma cholesterol and lipoprotein levels
- Absorption: Rapidly absorbed after oral administration; maximum plasma concentrations within 1 to 2 hours
- Metabolism: Extensively metabolized to active derivatives by cytochrome P450 3A4
- Excretion: Primarily eliminated in bile following hepatic and/or extra-hepatic metabolism
Indications
- Reduction of total cholesterol and LDL cholesterol in familial hypercholesterolaemia
- Treatment of hypertriglyceridaemia and dyslipidemia
- Prevention of cardiac ischaemic events in asymptomatic or mild to moderate coronary artery disease
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy and lactation
Contraindicated during pregnancy and breastfeeding; potential for serious adverse reactions
Precautions and warnings
Liver function tests should be performed before initiation of treatment; caution in patients with substantial alcohol consumption or history of liver disease
Special populations
- Geriatric: Higher plasma concentrations in elderly subjects
- Pediatric: Limited pharmacokinetic data available for the pediatric population
- Gender: No clinically significant difference in LDL-C reduction between men and women
Interactions
- Increased risk of myopathy with concurrent administration of specific drugs
- Reduced plasma concentrations when coadministered with antacid or colestipol
- Potential increase in plasma concentrations of digoxin, erythromycin, oral contraceptives
Overdose effects
Specific treatment not available; monitor liver function and serum CK levels
