Indications
- Reduction of total cholesterol and LDL cholesterol in patients with familial hypercholesterolaemia
- Treatment of mixed dyslipidemia
- Treatment of hypertriglyceridaemia
- Reduction of cardiac ischaemic events in patients with coronary artery disease
- Treatment of hypercholesterolemia associated with diabetes mellitus or renal transplantation
Mode of Action
- Selective inhibition of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels
- Increases hepatic LDL receptors for enhanced uptake and catabolism of LDL
Absorption
- Rapidly absorbed after oral administration
- Maximum plasma concentrations occur within 1 to 2 hours
Distribution
- Mean volume of distribution of Atorvastatin is approximately 381 liters
- Atorvastatin is 98% bound to plasma proteins
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives
- Cytochrome P450 3A4 plays a role in Atorvastatin metabolism
Excretion
- Eliminated primarily in bile following metabolism
- Mean plasma elimination half-life is approximately 14 hours
Adult Dosage
- Usually 10 mg once daily; may be increased to max. 80 mg once daily if necessary
Pediatric Dosage
- Child (10-18 years): Initially 10 mg once daily, increased if necessary to max. 20 mg once daily
Interaction with Drugs
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

- Liver function tests should be performed before initiation of treatment and periodically thereafter
Geriatric Use
- Plasma concentrations are higher in healthy elderly subjects
- Greater degree of LDL-lowering in the elderly patient population
Pediatric Use
- Pharmacokinetic data not available for the pediatric population
Gender Use
- No clinically significant difference in LDL-C reduction between men and women
Renal Insufficiency
- No influence on plasma concentrations or LDL-C reduction
- Dose adjustment not necessary
