Indications
- Topical prophylaxis against bacterial colonization and infection in burn wounds
- Topical antibacterial management of certain contaminated or infection-prone wounds other than burns
Pharmacology
- Mechanism of Silver Sulfadiazine's antibacterial action not fully elucidated
- Structural changes in bacterial cell membrane
- Weakening of cell wall membrane
- Reduction in viability of sensitive strains
- Silver sulfadiazine molecule dissociates and silver moiety is bound to bacterial cells
- Prevention of bacterial cell proliferation
- High ratio of silver sulfadiazine to bacterial DNA preventing division
- Sulfadiazine moiety provides bacteriostatic action against sensitive organisms
- Approximately 10% of sulfadiazine may be absorbed in adults
- 60-85% of absorbed amount excreted in urine
- Urinary sulfadiazine concentration in children with 13% body surface area burns was 31.8 mg/L
Dosage & Administration
- Cleansing burn wounds before application
- Apply over the burn wound
- Cover burn areas with Silver Sulfadiazine at all times
- Apply once to twice daily
- Apply to a thickness of approximately 1/16 inches or 1.5 mm
- Reapply to areas removed by patient activity
- Use dressings if necessary
- Reapply immediately after hydrotherapy
- Continue treatment until satisfactory healing or ready for grafting
Interaction
- Inactivation of enzymatic debriding agents
- Concomitant use with enzymatic debriding agents may be inappropriate
- Potentiation of action of oral hypoglycemic agents and phenytoin in patients with large area burns
- Monitoring of blood levels of oral hypoglycemic agents and phenytoin recommended
- Reported increase in leukopenia incidence with co-administration of Cimetidine
Contraindications
- Hypersensitivity to Silver Sulfadiazine or any ingredients
- Pregnant women approaching or at term
- Premature infants
- Newborn infants during the first 2 months of life
Side Effects
- Transient leukopenia
- Skin necrosis
- Erythema multiform
- Skin discoloration
- Burning sensation
- Rashes
- Interstitial nephritis
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly justified
- Excretion in human milk unknown
- Decision making for nursing mothers
Precautions & Warnings
- Impaired hepatic and renal functions may lead to drug accumulation
- Discontinuation may be weighed against therapeutic benefit
- Inactivation of topical proteolytic enzymes by silver
- Monitoring of serum sulfa concentrations in extensive burn wounds
- Careful monitoring of renal function
- Check urine for sulfa crystals
