Indications
- Topical prophylaxis against bacterial colonization and infection in burn wounds
- Topical antibacterial management of contaminated or infection-prone wounds
Pharmacology
- Antibacterial action mechanism not fully elucidated
- Structural changes in bacterial cell membrane
- Silver sulfadiazine molecule dissociates and binds to bacterial cells
- Inhibits bacterial cell proliferation by attaching to DNA
- Provides bacteriostatic action against sensitive organisms
- Approximately 10% of sulfadiazine absorbed in adults
- Up to 85% of absorbed amount excreted in urine
- Higher urinary sulfadiazine concentration in 13% body surface area burns children
Dosage & Administration
- Cream applied over burn wounds
- Cover burn areas with cream at all times
- Apply once to twice daily to a thickness of approximately 1/16 inches
- Reapply to areas removed by patient activity
- Continued until satisfactory healing occurs or ready for grafting
- Do not withdraw from therapeutic regimen if infection possibility remains
Interaction
- May inactivate enzymatic debriding agents
- Concomitant use with oral hypoglycemic agents and phenytoin may potentiate action
- Co-administration with cimetidine may increase leukopenia incidence
Contraindications
- Hypersensitivity to the product or its ingredients
- Pregnant women approaching or at term
- Premature infants
- Newborn infants during the first 2 months of life
Side Effects
- Transient leukopenia
- Skin necrosis
- Erythema multiform
- Skin discoloration
- Burning sensation
- Rashes
- Interstitial nephritis
Pregnancy & Lactation
- Pregnancy Category B
- Use during pregnancy only if clearly justified
- Not known if excreted in human milk
- Decision should consider importance of the drug to the mother
Precautions & Warnings
- Accumulation may occur with hepatic and renal impairment
- Consideration should be noted for inactivation of enzymes with silver
- Monitor serum sulfa concentrations in burn wounds involving extensive areas
- Carefully monitor renal function and check urine for sulfa crystals
