Indications

  • Topical prophylaxis against bacterial colonization and infection in burn wounds
  • Topical antibacterial management of contaminated or infection-prone wounds other than burns

Pharmacology

  • Silver Sulfadiazine's antibacterial action mechanism not fully elucidated
  • Structural changes in bacterial cell membrane occur after exposure to the drug
  • Silver sulfadiazine dissociates and the silver moiety is bound to bacterial cells
  • Silver moiety is attached to bacterial DNA preventing bacterial cell proliferation
  • Sulfadiazine moiety provides bacteriostatic action against sensitive organisms
  • Approximately 10% of sulfadiazine may be absorbed in adults
  • Approximately 60-85% of absorbed amount is excreted in urine

Dosage & Administration

  • Cleanse burn wounds before applying Silver Sulfadiazine
  • Apply Silver Sulfadiazine once to twice daily to a thickness of approximately 1/16 inches or 1.5 mm
  • Reapply cream to any areas removed by patient activity
  • Use dressings if necessary
  • Reapply immediately after hydrotherapy
  • Continue treatment until satisfactory healing occurs or until the burn site is ready for grafting

Interaction

  • Medizen may inactivate enzymatic debriding agents
  • Concomitant use of oral hypoglycemic agents and phenytoin may be inappropriate
  • Co-administration of Cimetidine may increase the incidence of leukopenia

Contraindications

  • Hypersensitivity to the drug or any other ingredients
  • Use is contraindicated in pregnant women approaching or at term, premature infants, or newborn infants during the first 2 months of life

  • Transient leukopenia
  • Skin necrosis
  • Erythema multiform
  • Skin discoloration
  • Burning sensation
  • Rashes
  • Interstitial nephritis

Pregnancy & Lactation

  • Pregnancy Category B
  • Use during pregnancy only if clearly justified
  • Not known if Silver Sulfadiazine is excreted in human milk
  • Safety and effectiveness not established in pediatric patients

Precautions & Warnings

  • Consider possibility of silver inactivating proteolytic enzymes
  • Discontinuation if hepatic and renal functions become impaired
  • Monitor serum sulfa concentrations in patients with extensive burn wounds
  • Monitor renal function and check urine for sulfa crystals
