Indications
- Topical prophylaxis against bacterial colonization in burn wounds
- Topical antibacterial management of contaminated wounds
Pharmacology
- Silver sulfadiazine's antibacterial action mechanism
- Structural changes in bacterial cell membrane
- Weakening of cell wall membrane
- Binding of silver moiety to bacterial cells
- Prevention of bacterial cell proliferation
Dosage & Administration
- Cleansing of burn wounds before application
- Application thickness of approximately 1/16 inches or 1.5 mm
- Reapplication after patient activity or hydrotherapy
- Continued treatment until satisfactory healing or grafting
Interaction
- Inactivation of enzymatic debriding agents
- Potentiation of oral hypoglycemic agents and phenytoin
- Increased incidence of leukopenia with cimetidine co-administration
Contraindications
- Hypersensitivity to silver sulfadiazine or any other ingredients
- Avoidance during pregnancy and on premature or newborn infants during the first 2 months of life
Side Effects
- Transient leukopenia
- Skin necrosis
- Erythema multiform
- Burning sensation
- Rashes
- Interstitial nephritis
Pregnancy & Lactation
- Pregnancy Category B
- Use in pregnancy justified if clearly needed
- Unknown excretion of Silver Sulfadiazine in human milk
- Consideration of drug discontinuation during nursing
Precautions & Warnings
- Potential inactivation of topical proteolytic enzymes
- Accumulation in impaired hepatic and renal function
- Monitoring of serum sulfa concentrations in extensive burn wounds
- Careful monitoring of renal function and detection of sulfa crystals in urine
