Indications
- To reduce total cholesterol and LDL cholesterol in patients with heterozygous and homozygous familial hypercholesterolaemia
- To reduce elevated cholesterol and triglycerides in patients with mixed dyslipidemia
- For the treatment of patients with elevated serum triglyceride levels in hypertriglyceridaemia
- For the treatment of patients with dysbetalipoproteinaemia
- To reduce cardiac ischaemic events in patients with coronary artery disease and elevated LDL-cholesterol level
- To reduce total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation
Mode of Action
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase and cholesterol synthesis in the liver
- Increases the number of hepatic LDL receptors on the cell surface for enhanced uptake and catabolism of LDL
Absorption
- Rapidly absorbed after oral administration
- Maximum plasma concentrations occur within 1 to 2 hours
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products
- In vitro inhibition of HMG-CoA reductase by ortho- and parahydroxylated metabolites is equivalent to that of Atorvastatin
Excretion
- Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
- Mean plasma elimination half-life of Atorvastatin in humans is approximately 14 hours
Dosage & Administration
- Adults: Usually 10 mg once daily; may be increased at intervals to max. 80 mg once daily
- Child (10-18 years): Initially 10 mg once daily, increased if necessary to usual max. 20 mg once daily
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, azole antifungals
- Altered plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives
Contraindications
- Hypersensitivity to any component of the medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy & Lactation
- Contraindicated during pregnancy

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment and periodically thereafter
- Discontinue if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in Special Populations
- Higher plasma concentrations in healthy elderly subjects than in young adults
- No clinically significant difference in LDL-C reduction between men and women
- Renal disease has no influence on the plasma concentrations or LDL-C reduction
- Dose adjustment in patients with renal dysfunction is not necessary
Overdose Effects
- Specific treatment not available, treated symptomatically and supportive measures instituted
- Liver function tests should be performed and serum CK levels monitored
