Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Very potent topical corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties.
Dosage
- Cream, Ointment:
- Apply a thin layer twice daily
- Treatment should be limited to 2 consecutive weeks
- Spray: Apply once or twice daily; total dose should not exceed 50 ml weekly
- Shampoo: Applied once a day to affected areas only; treatment should be limited to 4 consecutive weeks
- Scalp Solution: Apply once or twice daily to affected areas; total dose should not exceed 50 ml weekly
Administration
Route of administration: Cutaneous. Creams are appropriate for moist or weeping surfaces, ointments for dry, lichenified, or scaly lesions.
Interaction
Co-administered drugs that inhibit CYP3A4 can increase systemic exposure of corticosteroids.
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for infected skin lesions or dermatoses in children under one year of age
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes in the skin
- Reports of pigmentation changes and hypertrichosis
- Exacerbation of symptoms may occur
Pregnancy & Lactation
Limited data in pregnant women; caution advised during pregnancy and lactation
Precautions & Warnings
- Long-term continuous therapy to be avoided, particularly in infants and children
- Atrophic changes may occur on the face after prolonged use
- Care needed to prevent entry into the eyes if applied to the eye lids
- Hazardous in psoriasis for a number of reasons including rebound relapses and development of tolerance
Use in Special Populations
Long-term continuous therapy should be avoided in infants and children under 12 years of age
Overdose Effects
Acute overdosage is unlikely; chronic overdosage may lead to features of hypercortisolism
