Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Skin conditions not responding to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory, antipruritic, vasoconstrictive properties
- Inhibits late phase allergic reactions
- Reduces cytokine production
Dosage
- Apply twice daily for adults, elderly, and children over 1 year
- Super-high potency - limit to 2 consecutive weeks
- Maximum weekly dose: 50g/week
- Treatment for scalp limited to 4 consecutive weeks
Administration
Route: Cutaneous. Creams for moist/weeping surfaces, ointments for dry/lichenified/scaly lesions
Interaction
Inhibition of CYP3A4 can increase systemic exposure
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Hypersensitivity to the preparation
- Primarily infected skin lesions caused by fungi or bacteria
- Dermatoses in children under 1 year
Side Effects
- Prolonged use may lead to hypercorticism
- Local atrophic changes in the skin
- Pustular form of psoriasis possible
- Pigmentation changes, hypertrichosis
- Exacerbation of symptoms
Pregnancy & Lactation
Limited data on use in pregnant women. Caution in lactation
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Atrophic changes on the face after prolonged treatment
- Avoid application to eyelids to prevent glaucoma or cataract
- Hazardous in psoriasis for multiple reasons
- Use appropriate antimicrobial therapy for infected lesions
Use in Special Populations
Avoid long-term continuous therapy in children under 12 years
Overdose Effects
Acute overdosage unlikely, discontinue in case of chronic over-dosage or misuse
