Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream:
- 0.5 mg (0.05% w/w)
- Clobetasol Propionate Ointment:
- 0.5 mg (0.05% w/w)
- Clobetasol Propionate Scalp Application:
- 0.5 mg (0.05% w/w)
- Clobetasol Propionate Shampoo:
- 0.5 mg (0.05% w/w)
- Clobetasol Propionate Lotion:
- 0.5 mg (0.05% w/w)
- Clobetasol Propionate Spray:
- 0.5 mg (0.05% w/w)
Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Decreases mast cell density
- Reduces cytokine production
- Inhibits arachidonic acid metabolism
Dosage
- Cream, Ointment:
- Apply a thin layer twice daily to affected skin areas
- Maximum weekly dose: 50 gm/week
- Spray:
- Apply once or twice daily to affected areas of the scalp
- Total dose should not exceed 50 ml weekly
- Shampoo:
- Applied once a day to affected areas only
- Total dosage should not exceed 50 g per week
- Scalp Solution:
- Apply once or twice daily to affected areas of the scalp
- Total dose should not exceed 50 ml weekly
- General:
- Treatment with super-high potency corticosteroids should be limited to 2 consecutive weeks
Administration
- Route of Administration: Cutaneous
- Creams: Moist or weeping surfaces
- Ointments: Dry, lichenified or scaly lesions
Interaction
- Co-administration with drugs inhibiting CYP3A4 can increase systemic exposure
Contraindications
- Rosacea, acne vulgaris and perioral dermatitis
- Hypersensitivity to the preparation
- Dermatoses in children under one year of age
Side Effects
- Systemic absorption leading to hypercorticism
- Local atrophic changes in the skin
- Pustular form of psoriasis
- Pigmentation changes
- Hypertrichosis
Pregnancy & Lactation
- Limited data on use in pregnant women
- Caution during lactation
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Use on face may cause atrophic changes
- Avoid eye contact
- Hazardous in psoriasis
- Appropriate antimicrobial therapy when treating infected lesions
Use in Special Populations
- Adrenal suppression in children under 12 years
- Infants are more susceptible to atrophic changes
Overdose Effects
- Unlikely to occur acutely
- Discontinue in case of chronic over-dosage
