Indications
- Indicated as an adjunct to diet to reduce elevated total cholesterol
- LDL cholesterol, apolipoprotein B (Apo-B) and triglycerides levels
Pharmacology
- Selective inhibitor of HMG-CoA reductase
- Lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase
- Increases the number of hepatic LDL receptors for enhanced uptake and catabolism of LDL
Absorption
- Rapidly absorbed after oral administration
- Maximum plasma concentrations occur within 1 to 2 hours
- Extent of absorption increases in proportion to dose
Distribution
- Mean volume of distribution is approximately 381 liters
- 98% bound to plasma proteins
- Not expected to undergo enterohepatic recirculation
Metabolism
- Extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products
- Importance of metabolism by cytochrome P450 3A4
Excretion
- Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
- Mean plasma elimination half-life is approximately 14 hours
Dosage & Administration
- Adults usually take 10 mg once daily; may be increased to max. 80 mg once daily if necessary
- Children (10-18 years) initially take 10 mg once daily; increased to usual max. 20 mg once daily if necessary
Interaction
- Risk of myopathy increased with concurrent administration of certain drugs
- Plasma concentrations altered when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, or warfarin
Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy & Lactation
- Atorvastatin is contraindicated during pregnancy

Precautions & Warnings
- Liver function tests should be performed before initiation of treatment
- Used with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
Use in Special Populations
- Plasma concentrations are higher in healthy elderly subjects than in young adults
- No clinically significant difference in LDL-C reduction between men and women
- Renal disease has no influence on the plasma concentrations
- Hemodialysis is not expected to significantly enhance clearance
- In patients with chronic alcoholic liver disease, plasma concentrations are markedly increased
Overdose Effects
- Specific treatment is not available for atorvastatin overdose
- The patient should be treated symptomatically and supportive measures instituted
