Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions not responding to less potent steroids
- For adults, elderly, and children over 1 year
Composition
- Clobetasol Propionate Cream 0.05% w/w
- Clobetasol Propionate Ointment 0.05% w/w
- Clobetasol Propionate Scalp Application 0.05% w/w
- Clobetasol Propionate Shampoo 0.05% w/w
- Clobetasol Propionate Lotion 0.05% w/w
- Clobetasol Propionate Spray 0.05% w/w
Pharmacology
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Reduces mast cell density
- Inhibits arachidonic acid metabolism
Dosage
- Apply thin layer twice daily
- Super-high potency corticosteroid
- Limited to 2 consecutive weeks
- Maximum weekly dose 50gm/week
- Total dose applied should not exceed 50ml weekly

- Cutaneous route
- Creams for moist surfaces
- Ointments for dry or scaly lesions
Interaction
- Inhibition of CYP3A4
- Increased systemic exposure
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Primary cutaneous viral infections
- Hypersensitivity
- Not for primarily infected lesions
- Not for children under one year
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes
- Pustular form of psoriasis
- Pigmentation changes
- Hypertrichosis
Pregnancy & Lactation
- Limited data in pregnant women
- Caution during lactation
- Unknown excretion in human milk
Precautions & Warnings
- Avoid long-term therapy in infants and children
- Atrophic changes on the face
- Careful application near eyes
- Hazardous in psoriasis
- Use anti-microbial therapy when infected
Use in Special Populations
- Avoid long-term therapy in children under 12
- Children more susceptible to atrophic changes
Overdose Effects
- Acute overdosage unlikely
- Features of hypercortisolism with chronic use
