Description
- Zapitor is a selective inhibitor of HMG-CoA reductase.
- It is used to reduce elevated total cholesterol, LDL cholesterol, apolipoprotein B and triglycerides levels.
- It is indicated for reducing total cholesterol and LDL cholesterol in patients with familial hypercholesterolaemia, mixed dyslipidemia, hypertriglyceridaemia, dysbetalipoproteinaemia, and for reducing cardiac ischaemic events in patients with coronary artery disease.
- It is also used to reduce cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation.
- Atorvastatin, the active ingredient, lowers plasma cholesterol and lipoprotein levels by inhibiting HMG-CoA reductase and cholesterol synthesis in the liver.
- It increases the number of hepatic LDL receptors for enhanced uptake and catabolism of LDL.
- The recommended dosage ranges from 10 to 80 mg once daily for adults, and 10 to 20 mg for children aged 10-18 years.
- Zapitor should be taken with caution in patients with a history of liver disease or those who consume substantial quantities of alcohol.
- It is contraindicated during pregnancy and breastfeeding.
- The most frequent side effects related to Zapitor are constipation, flatulence, dyspepsia, and abdominal pain.
- This medicine should be stored in a dry place away from light and heat, and kept out of the reach of children.
Dosage & Administration
- Adults: Usually 10 mg once daily; may be increased up to max. 80 mg once daily if necessary.
- Children (10-18 years): Initially 10 mg once daily, increased if necessary to max. 20 mg once daily.
- The patient should be placed on a standard cholesterol-lowering diet before receiving Atorvastatin.
Indications
- Reduction of total cholesterol and LDL cholesterol in familial hypercholesterolaemia.
- Reduction of elevated cholesterol and triglycerides in mixed dyslipidemia.
- Treatment of patients with elevated serum triglyceride levels in hypertriglyceridaemia.
- For the treatment of dysbetalipoproteinaemia.
- To reduce cardiac ischaemic events in patients with coronary artery disease.
- To reduce total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with diabetes mellitus or renal transplantation.
Pharmacology
- Atorvastatin is rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours.
- Mean volume of distribution of Atorvastatin is approximately 381 liters.
- Atorvastatin is extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products
- Atorvastatin and its metabolites are eliminated primarily in bile following hepatic and/or extra-hepatic metabolism.
Interaction
- Increased risk of myopathy with concurrent administration of cyclosporine, fibric acid derivatives, niacin, erythromycin, and azole antifungals.
- Altered plasma concentrations when coadministered with antacid, colestipol, digoxin, erythromycin, oral contraceptives, and warfarin.
Contraindications
- Hypersensitivity to any component of this medication.
- Active liver disease or unexplained persistent elevations of serum transaminases.
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors.

Pregnancy & Lactation

- Breastfeeding is contraindicated due to the potential for serious adverse reactions.
Precautions & Warnings
- Liver function tests should be performed before the initiation of treatment and periodically thereafter.
- Use with caution in patients who consume substantial quantities of alcohol or have a history of liver disease.
- Therapy should be discontinued if markedly elevated CPK levels occur or myopathy is diagnosed or suspected.
Use in Special Populations
- Plasma concentrations are higher in healthy elderly subjects.
- Greater degree of LDL-lowering in the elderly patient population compared to younger adults.
- Pharmacokinetic data in the pediatric population are not available.
Overdose Effects
- Specific treatment is not available for atorvastatin overdose.
- Liver function tests should be performed and serum CK levels should be monitored.
