Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Decreases mast cell density
- Inhibits cytokine production
Dosage
- Apply a thin layer twice daily
- 2 consecutive weeks limit
- Maximum weekly dose: 50 gm
- Limited pediatric courses
- Apply required quantity of spray once or twice daily to affected scalp areas
- Treatment limited to 4 consecutive weeks for shampoo
Administration
Cutaneous. Creams for moist or weeping surfaces. Ointments for dry, lichenified, or scaly lesions.
Interaction
Inhibition of CYP3A4 can increase systemic exposure
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Primary cutaneous viral infections
- Hypersensitivity to the preparation
- Not indicated in primarily infected skin lesions
- Dermatoses in children under one year of age
Side Effects
- Hypercorticism with prolonged use
- Local atrophic changes
- Provocation of pustular form of psoriasis
- Pigmentation changes and hypertrichosis
- Hypersensitivity signs
Pregnancy & Lactation
Limited data in pregnant women. Caution during lactation.
Precautions & Warnings
- Avoid long-term continuous therapy, especially in infants and children
- Atrophic changes in facial areas
- Care to avoid entry into the eyes
- Hazardous in psoriasis
- Appropriate anti-microbial therapy for infected lesions
Use in Special Populations
Avoid long-term continuous therapy in children under 12 years
Overdose Effects
Acute overdosage is very unlikely. Chronic over-dosage may cause hypercortisolism
