Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive properties
- Inhibits late phase allergic reactions
- Reduces cytokine production
Dosage
- Cream and Ointment: Apply a thin layer twice daily
- Spray: Apply once or twice daily to affected areas
- Shampoo: Applied to dry scalp once a day
- Scalp Solution: Apply once or twice daily to affected areas of the scalp
Administration
- Route of administration: Cutaneous
- Creams are especially appropriate for moist or weeping surfaces
- Ointments are especially appropriate for dry, lichenified or scaly lesions
Interaction
- Co-administered drugs that can inhibit CYP3A4 may increase systemic exposure
Contraindications
- Rosacea, acne vulgaris and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions caused by fungi or bacteria
- Dermatoses in children under one year of age
Side Effects
- Prolonged use may result in systemic absorption
- Local atrophic changes in the skin
- Pustular form of psoriasis may be provoked
- Pigmentation changes and hypertrichosis
- Hypersensitivity reactions
Pregnancy & Lactation
- Limited data in pregnant women
- Topical administration to pregnant animals can cause foetal abnormalities
- Caution during lactation
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Atrophic changes may occur on the face after prolonged treatment
- Avoid application to eye lids to prevent entry into the eye
- Risk of generalised pustular psoriasis and local or systemic toxicity
- Proper anti-microbial therapy for infected lesions
Use in Special Populations
- Adrenal suppression can occur in infants and children under 12 years of age
Overdose Effects
- Acute overdosage is very unlikely to occur
- Chronic over-dosage can lead to hypercortisolism
