Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Very potent topical corticosteroid with anti-inflammatory, antipruritic, and vasoconstrictive properties. It decreases mast cell density, chemotaxis, and activation of eosinophils, and inhibits cytokine production.
Dosage
- Adults, elderly and children over 1 year: Apply a thin layer of Clobetasol Propionate Cream or Ointment to the affected skin areas twice daily and rub in gently and completely.
- Clobetasol Propionate scalp solutions should not exceed 50 ml weekly.
Administration
Cutaneous application for moist or weeping surfaces (creams) and dry, lichenified, or scaly lesions (ointments).
Interaction
Co-administered drugs that inhibit CYP3A4 can lead to increased systemic exposure to corticosteroids.
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Primary cutaneous viral infections
- Dermatoses in children under one year of age
Side effects
- Prolonged use or treatment of extensive areas can result in systemic absorption
- Local atrophic changes in the skin with prolonged and intensive treatment
- Reports of pigmentation changes and hypertrichosis
Pregnancy lactation
Limited data in pregnant women; caution in pregnancy and lactation due to potential risks to the fetus or nursing infant.
Precautions warnings
- Long-term continuous topical therapy should be avoided, particularly in infants and children
- Atrophic changes may occur after prolonged treatment on the face
- Care needed to prevent entry into the eyes
Use special populations
Long-term topical corticosteroid therapy should be avoided in infants and children under 12 years
Overdose effects
Chronic over-dosage may lead to hypercortisolism features; discontinue topical steroid in case of misuse
