Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Dermatoses not responding to less potent steroids
- For adults, elderly, and children over 1 year
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Very potent topical corticosteroid with anti-inflammatory, antipruritic and vasoconstrictive properties. Inhibits late phase allergic reactions, reduces cytokine production, and inhibits arachidonic acid metabolism.
Dosage
- Apply thin layer of cream or ointment twice daily for adults, elderly, and children over 1 year
- Use spray once or twice daily on scalp, maximum weekly dose not to exceed 50 ml
- Apply shampoo to dry scalp once a day, limited to 4 consecutive weeks
- Scalp solution applied once or twice daily, total dose not to exceed 50 ml weekly
Administration
Route of administration: Cutaneous. Creams suitable for moist or weeping surfaces. Ointments suitable for dry, lichenified, or scaly lesions.
Interaction
Co-administered drugs inhibiting CYP3A4 can lead to increased systemic exposure of corticosteroids.
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for infected skin lesions caused by fungi or bacteria
- Not for use in children under one year of age
Side Effects
- Prolonged use or treatment of extensive areas may result in systemic absorption
- May cause local atrophic changes in the skin
- Rare instances of provoking the pustular form of psoriasis
- Reports of pigmentation changes and hypertrichosis
Pregnancy & Lactation
Limited data in pregnant women, caution in lactating women.
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Prolonged treatment on face may cause atrophic changes in the skin
- Care needed to prevent entry into the eyes
- Hazardous in psoriasis for a number of reasons
Use in Special Populations
- Avoid long-term continuous therapy in infants and children under 12 years
- Children more susceptible to atrophic changes
Overdose Effects
Acute overdosage unlikely, features of hypercortisolism may occur in chronic over-dosage.
