Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions not responding satisfactorily to less potent steroids
Composition
- 0.05% w/w Clobetasol Propionate Cream
- 0.05% w/w Clobetasol Propionate Ointment
- 0.05% w/w Clobetasol Propionate Scalp Application
- 0.05% w/w Clobetasol Propionate Shampoo
- 0.05% w/w Clobetasol Propionate Lotion
- 0.05% w/w Clobetasol Propionate Spray
Pharmacology
- Very potent topical corticosteroid
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibits late phase allergic reactions
- Decreases mast cell density
- Inhibits arachidonic acid metabolism
Dosage
- Apply a thin layer of cream or ointment twice daily
- Enhance effect by occluding treatment area with polythene film
- Limit treatment to 2 consecutive weeks
- Maximum weekly dose should not exceed 50 gm/week
- For children, limit courses to five days and review weekly
- Apply required quantity of spray to affected scalp areas once or twice daily
- Total dose applied should not exceed 50 ml weekly
- Apply shampoo to dry scalp once a day to affected areas only
- Massaged into the scalp using fingers if necessary
- Limit treatment to 4 consecutive weeks
- Apply required quantity of Clobetasol Scalp Solution once or twice daily
- Total dose applied should not exceed 50 ml weekly
- Use for brief periods only
Administration
- Route of administration: Cutaneous
- Creams for moist or weeping surfaces
- Ointments for dry, lichenified or scaly lesions
Interaction
- Co-administered drugs inhibiting CYP3A4 increase systemic exposure
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for infected skin lesions or perianal/genital pruritus
- Dermatoses in children under one year of age
Side Effects
- Prolonged use can result in systemic absorption
- Local atrophic changes in the skin
- Rare instances of provoking pustular form of psoriasis
- Reports of pigmentation changes and hypertrichosis
Pregnancy & Lactation
- Limited data for pregnant women
- Caution in lactating women
Precautions & Warnings
- Avoid long-term continuous therapy, especially in infants and children
- Atrophic changes may occur, especially on the face
- Care needed to prevent entry into the eyes
- Hazardous in psoriasis due to rebound relapses and tolerance
- Use anti-microbial therapy for infected lesions
Use in Special Populations
- Avoid long-term therapy in infants and children
Overdose Effects
- Acute overdosage is unlikely
- Chronic overdosage may lead to hypercortisolism
