Indications
- Short courses treatment of recalcitrant eczemas
- Neurodermatoses
- Psoriasis (excluding widespread plaque psoriasis) where secondary bacterial or fungal infection is present
- Other inflammatory conditions which do not respond satisfactorily to less active steroids
Pharmacology
- Very potent corticosteroid
- Used to treat severe inflammatory skin disorders such as eczema and psoriasis
- Neomycin Sulphate is an antibiotic of the aminoglycoside type
- Nystatin is an antifungal that kills fungi and yeasts by interfering with their cell membranes
- Mechanism of topical steroids like Clobetasol is unclear
- Main effects include anti-inflammatory activity, vasoconstriction, and decrease in collagen synthesis
Dosage & Administration
- Adults: Apply sparingly once or twice daily until improvement occurs
- Treatment should not be continued for more than 7 days without medical supervision
- Elderly: Caution should be exercised in cases where a decrease in renal function exists
- Children: Suitable for use in children (2 years and over) at the same dose as adults
Interaction
- Neomycin Sulphate can intensify and prolong the respiratory depressant effects of neuromuscular blocking agents
- Systemic exposure to Neomycin Sulphate is expected to be minimal
Contraindications
- Contraindicated in rosacea, acne vulgaris and perioral dermatitis
- Primary cutaneous viral infection (e.g., herpes simplex, chicken pox)
- Hypersensitivity to the preparation
Side Effects
- Prolonged use can result in systemic absorption and hypercortisolism
- Local atrophic changes in the skin such as thinning, striae, and dilatation of the superficial blood vessels
- Pigmentation changes and hypertrichosis with topical steroids
Pregnancy & Lactation
- Topically applied Neomycin's effect in pregnancy and lactation unknown
- Neomycin can cross the placenta and may pose a theoretical risk of fetal toxicity
- Safety of Clobetasol Propionate not established in lactating mothers
Precautions & Warnings
- Long-term continuous topical therapy should be avoided particularly in infants and children
- Care to ensure the preparation does not enter the eye if applied to the eyelids
Overdose Effects
- Acute overdosage is very unlikely to occur
- Features of hypercortisolism may appear in case of chronic overdosage or misuse
