Medicine Details

Indications

  • Short courses treatment of recalcitrant eczemas.
  • Neurodermatoses.
  • Psoriasis (excluding widespread plaque psoriasis) where secondary bacterial infection or fungal infection is present, suspected or likely to occur.
  • Other inflammatory conditions which do not respond satisfactorily to less active steroids.

Pharmacology

  • Very potent corticosteroid.
  • Treats severe inflammatory skin disorders such as eczema and psoriasis.
  • Contains antibiotic (Neomycin Sulphate) of the aminoglycoside type.
  • Contains antifungal (Nystatin).
  • Unclear mechanism of action for topical steroids like Clobetasol.

Dosage & Administration

  • Adults: Apply sparingly to the affected area once or twice daily until improvement occurs.
  • Enhanced anti-inflammatory effect with occlusion in very resistant lesion.
  • Treatment should not be continued for more than 7 days without medical supervision.
  • Recommended review of patient's condition if treatment is longer than 4 weeks.
  • Suitable for use in elderly and children over 2 years old.

Interaction

  • Neomycin Sulphate can intensify and prolong the respiratory depressant effects of neuromuscular blocking agents.
  • Systemic exposure to Neomycin Sulphate expected to be minimal with recommended use.
  • No hazardous interactions reported with Clobetasol Propionate or Nystatin.

Contraindications

  • Contraindicated in rosacea, acne vulgaris, and perioral dermatitis.
  • Contraindicated in primary cutaneous viral infections (e.g., Herpes simplex, chicken pox).
  • Contraindicated in hypersensitivity to the preparation.

Side Effects

  • Prolonged use or treatment of extensive areas can result in systemic absorption.
  • More likely to occur in infants and children and if occlusive dressings are used.
  • Prolonged and intensive treatment may cause local atrophic changes in the skin.
  • Reports of pigmentation changes and hypertrichosis with topical steroids.

Pregnancy & Lactation

  • Little information available about the possible effect of topically applied Neomycin in pregnancy and lactation.
  • Neomycin present in maternal blood can cross the placenta and may pose a theoretical risk of foetal toxicity.
  • Safety of Clobetasol Propionate not established in lactating mothers.

Precautions & Warnings

  • Long-term continuous topical therapy should be avoided, particularly in infants and children.
  • Care needed to prevent the preparation from entering the eye if applied to the eyelids.
  • Thorough washing of the affected eye with water if the medication enters the eye.

Overdose Effects

  • Acute overdosage is very unlikely to occur.
  • No overdose-related problems reported.
  • Topical steroids should be discontinued gradually in case of chronic overdosage or misuse.
