Indications
- Relief of inflammatory and pruritic manifestations of corticosteroid responsive eczema/dermatitis
Pharmacology
- Fluticasone propionate is a glucocorticoid with high topical anti-inflammatory potency
- Low HPA-axis suppressive activity after dermal administration
- Therapeutic index greater than most commonly available steroids
- High degree of selectivity for the glucocorticoid receptor
- Strong affinity for human glucocorticoid receptors
- Weak affinity for the progesterone receptor
- Virtually no affinity for the mineralocorticoid, estrogen, or androgen receptors
- Half-life of approximately 10 hours for the glucocorticoid-receptor complex
Dosage & Administration
- Cream: Apply a thin layer once daily
- Ointment: Apply a thin layer twice daily

Contraindications
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Primary cutaneous viral infections (e.g., Herpes simplex, chicken pox)
- Hypersensitivity to any of the ingredients
- Perianal and genital pruritus
- Not indicated in the treatment of primarily infected skin lesions caused by fungi or bacteria in children under one year of age
Side Effects
- Local burning and pruritus
- Signs of hypersensitivity require immediate cessation of application
- Atrophic changes in the skin
- Secondary infection particularly with occlusive dressings or when skin folds are involved
- Allergic contact dermatitis
- Exacerbation of dermatoses
- Systemic absorption leading to features of hypercorticism
- Infants and children more susceptible to systemic toxicity
- Napkin acting as an occlusive dressing in infants
Pregnancy & Lactation
- Consideration of expected benefit to mother versus possible risk to fetus during pregnancy
- Excretion into human breast milk not investigated
- Therapeutic benefits versus potential hazards must be weighed during breastfeeding
Precautions & Warnings
- Low propensity for systemic absorption
- Prolonged application to large areas in infants may lead to adrenal suppression
- Proportionally larger absorption in children
- Atropic changes on the face after prolonged treatment
- Use of appropriate antimicrobial therapy for infected inflammatory lesions
- Encouragement of bacterial infection with occlusive dressing, hence skin cleansing before fresh dressing is applied
Overdose Effects
- Acute overdosage very unlikely
- Chronic overdosage may lead to hypercorticism features
- Discontinuation of application in case of overdosage
- Extremely low oral bioavailability
