Indications
- Cardiovascular disorders
- Anaesthetic premedication
- Anticholinesterase and mushroom poisoning
- Control of muscarinic side-effects of neostigmine
Pharmacology
- Binds to and inhibits muscarinic acetylcholine receptors
- Produces anticholinergic effects
- Blocks muscarinic receptors in peripheral tissues
- Produces mydriatic and cycloplegic effect
Dosage & Administration
- Initial intravenous doses of 0.5 to 1.0 mg for cardiovascular disorders
- Subcutaneous or intramuscular injection for anaesthetic premedication
- Variety of doses for paediatric premedication
- Large doses for anticholinesterase and mushroom poisoning
- Intravenous injection for muscarinic side-effects of neostigmine
- Particularly susceptible in infants and children
Interaction
- Additive anticholinergic action with other drugs
- Delays gastric emptying
- Accentuates reduction of lower oesophageal pressure when combined with metoclopramide
Contraindications
- Glaucoma
- Chronic lung disease
- Unstable cardiac rhythm
- Prostatic hypertrophy
- Reflux oesophagitis
- Paralytic ileus or pyloric stenosis
- Generally contraindicated in the elderly
Side Effects
- Dry mouth
- Difficulty in swallowing and talking
- Thirst
- Reduced bronchial secretions
- Mydriasis
- Cycloplegia
- Photophobia
- Flushing
- Dry skin
- Bradycardia followed by tachycardia
- Palpitations
- Arrhythmias
- Difficulty in micturition
- Reduction in GIT tone and motility leading to constipation
- Occasional vomiting
Pregnancy & Lactation
- Negligible effects on human uterus
- Not teratogenic
- Small quantities in breast milk
- Reported impairment of milk production
Precautions & Warnings
- Caution in children and geriatric patients
- Avoid in patients with diarrhoea or fever
- Increased susceptibility in Down's syndrome patients
- Care in conditions characterized by tachycardia
- Caution in acute myocardial infarction, hypertension, and fever
- Risk of mental confusion, especially in the elderly
- Reduced bronchial secretions may lead to mucous plug formation
- Gradual dosage increases and withdrawal for parkinsonism treatment
