Indications
- Symptomatic treatment of chronic stable angina pectoris
- Coronary artery disease patients with normal sinus rhythm
- In patients unable to tolerate or with a contra-indication to the use of beta-blockers
- In combination with beta-blockers in patients inadequately controlled with an optimal beta-blocker dose
- Heart rate > 60 bpm
Pharmacology
- Pure heart rate lowering agent
- Selective inhibition of the cardiac pacemaker I f current
- Regulation of heart rate
- Decreases the cardiac workload and oxygen consumption
- Prolongs diastole allowing increased perfusion of coronary arteries
- Increases oxygen supply to the heart
- No effect on intra-atrial, atrioventricular or intraventricular conduction times
- No effect on myocardial contractility or ventricular repolarization
Dosage & Administration
- Usual recommended starting dose of 5 mg twice daily
- May be increased to 7.5 mg twice daily after 3-4 weeks of treatment
- Usual dose is 1 tablet in the morning and 1 tablet in the evening during meals
- Adjust dose downwards if heart rate decreases persistently below 50 bpm
- Lower starting dose for elderly patients
- No dose adjustment for patients with renal insufficiency and creatinine clearance above 15ml/min
- Caution in patients with moderate hepatic impairment
- Not recommended for use in children and adolescents
Interaction
- Not recommended with QT wave prolonging medicinal products
- Close cardiac monitoring if concomitant use is necessary
Contraindications
- History of hypersensitivity to Ivabradine or any of the excipients
- Resting heart rate below 60 bpm before treatment
- Cardiogenic shock
- Acute myocardial infarction
- Severe hypotension (<90/50 mmHg)
- Severe hepatic insufficiency
- Sick sinus syndrome
- Sino-atrial block
- Heart failure
- Pacemaker dependent
- Unstable angina
- 3rd degree AV block
- Combination with strong cytochrome P-450 3A4 inhibitors

- Blurred vision
- Bradycardia
- 1st degree AV block
- Ventricular extrasystoles
- Headaches
- Dizziness
Pregnancy & Lactation
- Contra-indicated during pregnancy
- Excreted in milk
- Contra-indicated during breast-feeding
Precautions & Warnings
- Mild to moderate hypotension
- Atrial fibrillation
- Patients with congenital QT syndrome or treated with QT wave prolonging medicinal products
- Moderate hepatic insufficiency
- Severe renal insufficiency
