Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Other skin conditions which do not respond satisfactorily to less potent steroids

Pharmacology
- Anti-inflammatory
- Antipruritic
- Vasoconstrictive
- Inhibition of late phase allergic reactions
- Reduction of mast cell density
- Inhibition of cytokine production
Dosage
- Application twice daily
- Enhancement with occlusion
- Super-high potency - limited to 2 consecutive weeks
- Recommended dosage limited to 50 gm/week
Administration
- Route of administration: Cutaneous
- Creams suitable for moist or weeping surfaces
- Ointments suitable for dry, lichenified or scaly lesions
Interaction
- Inhibition of CYP3A4 metabolism
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions
- Dermatoses in children under one year of age
Side Effects
- Systemic absorption leading to hypercorticism
- Local atrophic changes in the skin
- Provocation of pustular form of psoriasis
- Pigmentation changes and hypertrichosis
Pregnancy & Lactation
- Limited data from use in pregnant women
- Unknown excretion in human milk
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Potential atrophic changes on the face
- Risk of glaucoma or cataract if applied to the eyelids
- Risk in psoriasis treatment
- Appropriate anti-microbial therapy for inflammatory lesions
Use in Special Populations
- Adrenal suppression in children
- Susceptibility of children to atrophic changes

- Features of hypercortisolism in chronic over-dosage
