Indications
- Short courses treatment of recalcitrant eczemas
- Neurodermatoses
- Psoriasis (excluding widespread plaque psoriasis) where secondary bacterial infection or fungal infection is present, suspected or likely to occur
- Other inflammatory conditions which do not respond satisfactorily to less active steroids
Pharmacology
- Clobetasol Propionate is a potent corticosteroid for severe inflammatory skin disorders
- Neomycin Sulphate is an antibiotic of the aminoglycoside type used to treat bacterial infections
- Nystatin is an antifungal that kills fungi and yeasts by interfering with their cell membranes
- Mechanism of topical steroids like Clobetasol, in general, is unclear, but has nonspecific anti-inflammatory response and vasoconstriction effect
- Neomycin binds to the ribosomal sub-units of susceptible bacteria and inhibits protein synthesis
- Nystatin acts by binding to sterols in the cell membrane of the fungus leading to membrane permeability
Dosage & Administration
- Adults: Apply sparingly once or twice daily to the affected area until improvement occurs
- In very resistant lesions, enhance anti-inflammatory effect by occluding the treatment area
- Caution in elderly with decreased renal function and potential systemic absorption of Neomycin Sulphate
- Children (2 years and over) can use the same dose as adults
Interaction
- Neomycin can intensify and prolong the respiratory depressant effects of neuromuscular blocking agents following systemic absorption
- No hazardous interactions reported with Clobetasol Propionate or Nystatin
Contraindications
- Rosacea, acne vulgaris, and perioral dermatitis
- Primary cutaneous viral infection (e.g. Herpes simplex, chicken pox)
- Hypersensitivity to the preparation
Side Effects
- Prolonged use or treatment of extensive areas can result in systemic absorption and features of hypercortisolism
- More likely to occur in infants and children, especially with occlusive dressings
- Prolonged and intensive treatment may cause local atrophic changes
- Reports of pigmentation changes and hypertrichosis with topical steroids
Pregnancy & Lactation
- Limited information on the effect of topically applied Neomycin in pregnancy and lactation
- Neomycin can cross the placenta and pose a theoretical risk of foetal toxicity
- Safety of Clobetasol Propionate not established in lactating mothers
Precautions & Warnings
- Long-term continuous topical therapy should be avoided, particularly in infants and children
- Care needed to prevent entry into the eyes, as glaucoma might result
- If this medication enters the eye, thoroughly wash with water
Overdose Effects
- Acute overdosage is very unlikely to occur
- No reported overdose-related problems yet
- Chronic overdosage or misuse can lead to hypercortisolism features
