Indications
- Treatment of bradyarrhythmias
- Prevention and treatment of arrhythmias associated with anaesthesia
- Treating drug-induced arrhythmias
- Managing bradycardia of acute myocardial infarction
- Anaesthetic premedication to reduce salivary and bronchial secretions
- Control of muscarinic side-effects of neostigmine
Pharmacology
- Binds to and inhibits muscarinic acetylcholine receptors
- Anticholinergic effects
- Competitively blocks muscarinic receptors in peripheral tissues
- Reduces secretions and relaxes smooth muscles
- Has mydriatic and cycloplegic effect
Dosage & Administration
- Typical initial doses in the range of 0.5 to 1.0 mg intravenously
- Recommended dosage intervals
- Pediatric premedication doses based on weight
- Used in very large doses for anticholinesterase and mushroom poisoning
- Control of muscarinic side-effects with neostigmine
- Toxic effects in infants and children
Interaction
- Additive anticholinergic action with other drugs
- Addition of metoclopramide
- Delays gastric emptying
- May delay onset of action of other drugs

- Reflux esophagitis

- Dryness of mouth
- Difficulty in swallowing and talking
- Thirst
- Reduced bronchial secretions
- Mydriasis
- Cycloplegia
- Photophobia
- Flushing
- Dryness of skin
- Bradycardia followed by tachycardia
- Palpitations and arrhythmias
- Difficulty in micturition
- Constipation
- Vomiting
Pregnancy & Lactation
- Negligible effects on human uterus
- Not teratogenic
- Small quantities in breast milk
- Reported to impair milk production
Precautions & Warnings
- Caution in children and geriatric patients
- Advisable to be cautious in patients with diarrhea
- Avoid in high ambient temperatures
- Use cautiously in conditions characterized by tachycardia
- May cause mental confusion, especially in the elderly
- Care needed in patients with acute myocardial infarction and hypertension
- Increased susceptibility in persons with Down's syndrome
- Increased risk in conditions characterized by tachycardia
