Indications
- Cardiovascular disorders
- Anaesthetic premedication
- Anticholinesterase and mushroom poisoning
- Control of muscarinic side-effects of neostigmine
- Management of bradycardia of acute myocardial infarction
Pharmacology
- Binds to and inhibit muscarinic acetylcholine receptors
- Produces anticholinergic effects
- Abolishes bradycardia
- Reduces heart block due to vagal activity
- Relaxes smooth muscles in the bronchi and gut
- Reduces glandular secretions
- Has mydriatic and cycloplegic effect
Dosage & Administration
- Initial doses in the range of 0.5 to 1.0 mg intravenously
- Suitable paediatric premedication doses based on children's weight
- Usage in anticholinesterase and mushroom poisoning with very large doses
- Infants and children are particularly susceptible to toxic effects of atropine
Interaction
- Additive with other drugs having anticholinergic activity
- May delay the onset of action of other drugs
- Addition of metoclopramide accentuates the reduction of lower oesophageal pressure produced by atropine premedication
Contraindications
- Glaucoma
- Chronic lung disease
- Unstable cardiac rhythm
- Prostatic hypertrophy
- Reflux oesophagitis
- Paralytic ileus or pyloric stenosis
- Generally in the elderly
Side Effects
- Dryness of the mouth
- Difficulty in swallowing and talking
- Thirst
- Reduced bronchial secretions
- Mydriasis with loss of accomodation
- Photophobia
- Flushing
- Dryness of skin
- Transient bradycardia followed by tachycardia with palpitations and arrhythmias
- Difficulty in micturition
- Reduction in the tone and motility of GIT leading to constipation
- Occasional vomiting
Pregnancy & Lactation
- Negligible effects on the human uterus
- Not teratogenic
- Small quantities appear in breast milk
- Reported to impair milk production
Precautions & Warnings
- Caution in children and geriatric patients
- Cautious in patients with diarrhoea or fever
- Increased susceptibility in persons with Down's syndrome
- Care required in conditions characterized by tachycardia
- May cause mental confusion, especially in the elderly
- Reduced bronchial secretion may be associated with the formation of mucous plugs
